Generally, everyone will need to provide the following information:
- Full Name
- Date of Birth
- Phone Number
- Full Address (and directions if needed)
- Pay Source (who’s paying for the ride)
- Requested pick-up time and appointment time if applicable
- Destination
- Number of people going
- Will anyone be using a mobility device – wheelchair, scooter or walker…..
If the requested ride is to be billed to Medical Assistance or a managed care provider we will also need:
- Member Number
- County Responsible
- Case manager, social worker or financial worker
- Destination information including street address and phone number
- Verification of authorization for the ride
Dispatchers will need verify eligibility prior to placing a ride.
